• Application for Reserved Name - Available from the Secretary of State to reserve a name prior to formation of a particular business. You can also search for available names and file your application online.
• Federal Employer Identification Number (FEIN) - Register with the IRS for FEIN using paper form Form SS-4 or apply online.
• Registration of a new entity - New business entities can register online with the Kentucky Secretary of State or via the appropriate form for their business structure.
• Kentucky Tax Registration (10A100) - Basic Kentucky tax registration can be completed online or via the Kentucky Tax Registration Application (10A100). Additional tax registrations may be required based on your industry, for more information visit the Kentucky Department of Revenue.
• Register for Unemployment Insurance Tax - Employers can either register a new account or can reinstate an old account online.
• Workers' Compensation Self Insurance Application - Employers are required to register for Workers’ Compensation Insurance, which is typically handled by your insurance provider. For those businesses which self insure forms SI-01, SI-02, and SI-02 Attachment must be completed.
