• Application for Reserved Name - Available from the Secretary of State to reserve a name prior to formation of a particular business. You can also search for available names and file your application online.
• Federal Employer Identification Number (FEIN) - Register with the IRS for FEIN using paper form Form SS-4 or apply online.
• Registration of a new entity - New business entities can register online with the Kentucky Secretary of State or via the appropriate form for their business structure.
• ​Basic Kentucky tax registration can be completed online. Additional tax registrations may be required based on your industry, for more information visit the Kentucky Department of Revenue.
• Register for Unemployment Insurance Tax - Employers can either register a new account or can reinstate an old account online.
• Workers' Compensation Self Insurance Application - Employers are required to register for Workers’ Compensation Insurance, which is typically handled by your insurance provider. For those businesses which self insure forms SI-01, SI-02, and SI-02 Attachment must be completed.
